I think he probably did a clever switch. However, it was never satisfactorily explained how it happened in the book, either. Beyond Sherlock going, "You just thought you saw both of us fall off the cliff, Watson. It was only Moriarty." Then, it also opens up my theory that the Moriarty we saw on the show is not the real Moriarty.
