Hmm, I do see what you're saying, because it did seem like we are supposed to focus on Dean's pain, and it's tantamount to Sam's issues, so forget them, but...I feel like this was a realization ep for Sam.
Dean is doing what he always does--lashing out because he's hurt deep down, because he can't separate the idea that Sam wanted to leave their lifestyle/John's control from the idea that Sam wanted to leave him. But the key to this ep, I thought, was Sam realizing that that's how Dean perceives things.
It kind of does marginalize Sam's feelings some, but also, I think it's setting him up to do something really huge for Dean. Of which I approve.
