To me it still feels like an entire episode was missing going into this one. There really should have been more set up.
Remember the early days of the show when Sam and Dean would drive around in the Impala and over explain the plot points? A few minutes of that early in the episode really wouldn't have hurt either.
So yeah, I'm pretty much feeling like you are about the whole thing.
