For me, it turned around after Dean told Sam he was more like his father than Dean could ever be and for Sam to take it however he wanted to take it.

I think the main factor that seperates the two is that Sam and John both sacrificed everything selfishly. Although they began with good intentions, (the road to hell yadda yadda), toward the end, Sam's motivation seemed to be about proving that either he wasn't a "freak" or he was a freak/monster/special for a reason. (He went ahead with killing Lilith after she called him a freak & reiterating what Sam thought Dean said on the message.)

From what I've seen, Dean always seemed to feel remorse, but summed it up as his familial duty, until the aforementioned episode where it appeared to click for him that it was all a farce.
